Informed Consent
Informed Consent for Counselling Services
Living Practice Counselling
This document serves as the Informed Consent for the counseling services provided by Living Practice Counselling. It outlines important information about the nature of counseling, the therapeutic process, confidentiality, and your rights as a client. By engaging in counseling services, you acknowledge that you have read, understood, and agreed to the terms and conditions described herein.
1. Nature of Counselling
Counseling is a collaborative process where you and your counselor work together to explore personal challenges, set goals, and identify solutions. The goal of counseling is to promote self-awareness, emotional growth, and positive change. While the process can lead to significant personal development, it may also bring up uncomfortable emotions, and progress can vary from person to person.
Types of Services Offered:
- Solution-Focused Brief Therapy (SFBT)
- ACT Therapy
- Addiction Counseling
- Family Therapy
- Counseling for Anxiety and Depression
2. Confidentiality
Confidentiality is a fundamental aspect of the counselling relationship, and all personal information shared during sessions will be kept private, with the following exceptions:
- If there is a risk of serious harm to yourself or others.
- If there is suspected abuse or neglect of a child, elder, or vulnerable person.
- If required by law or court order.
- If you provide written consent for information to be shared with another party (e.g., a medical provider).
In situations where confidentiality must be breached, we will make every effort to inform you before any action is taken, unless it is not feasible or safe to do so.
3. Records and Data
Counseling sessions may involve the creation of notes and records to document your treatment. These records are confidential and stored securely. You have the right to request access to your records, except in situations where doing so may cause harm.
4. Fees and Payment
Fees for counselling services will be discussed and agreed upon before starting therapy. Payment is due at the time of service unless otherwise arranged. We accept various payment methods, which will be discussed during the initial consultation.
If you need to cancel or reschedule an appointment, we request at least 24 hours’ notice. Failure to provide adequate notice may result in a cancellation fee.
5. Telehealth Services
If sessions are conducted via telehealth (phone, video calls), confidentiality and security are still prioritized. However, clients should be aware of the limitations of technology and privacy when using online communication tools.
6. Client Rights
As a client, you have the following rights:
- To participate in the development of your treatment goals.
- To ask questions about any aspect of the counselling process.
- To request changes in the counselling approach or to seek a second opinion.
- To discontinue counseling at any time, with the understanding that doing so may impact your progress.
- To access your personal records upon request, unless disclosure is deemed harmful to your well-being.
7. Risks and Benefits
Counselling has the potential for both positive and challenging experiences. You may feel uncomfortable discussing difficult emotions or past events, but this is part of the therapeutic process. Positive benefits may include improved emotional well-being, better coping skills, enhanced relationships, and personal growth. However, no specific outcomes or timeframes can be guaranteed.
8. Therapeutic Relationship
The relationship between counsellor and client is professional and collaborative. It is important to understand that this relationship is bound by ethical guidelines and professional standards, and it is not a friendship or social relationship.
9. Termination of Therapy
You have the right to discontinue therapy at any time. However, we encourage a discussion regarding termination to ensure you have the appropriate support in place moving forward. Your counsellor may also recommend the termination of therapy if it is believed that services are no longer beneficial or if referrals to other services are necessary.
10. Contact Information
You are encouraged to reach out if you have any questions, concerns, or would like to discuss any aspect of your treatment.
